How to make an appointment:

The staff at Care Center Rehabilitation and Pain Management is available for evaluations and treatment management. CCRPM staff will work in partnership with the referring physician to develop a plan that is best for you, the patient.

We consider it a great privilege that you have entrusted us with your care. We will do all we can to assure you that the right choice was made in treating at Care Center. We make every effort to schedule evaluations as soon as possible.

Patient care is provided through a multidisciplinary/interdisciplinary program that works to provide the best care possible by drawing together specialists from different fields to arrive at a specialized treatment plan developed just for your needs.

It is best if you are referred from your primary care physician. You will need a prescription written by your Primary Treating Physician requesting evaluations and treatment, with an indication of type of program desired. If your physician is not clear about our programs, we will be happy to get that information to him/her immediately. To make an appointment, simply call (818) 784-0990 or toll free (800) 301-CARE between 8 AM and 5:00 PM Monday through Friday except major holidays.

Once you have made an appointment:

The next step is to make sure your medical records are available at your first appointment. If you have seen any other physicians for your pain problem or injury, and have had any diagnostic tests, it is very important that you request that these results be forwarded to our office as soon as possible, or bring them with you at the time of your initial evaluations. This will help us in evaluating your problem and prescribing the very best program recommendations for you. For your convenience, these records can be faxed to our office at (818) 784-9069 or e-mailed to us at info@carectr.com.

In order to serve you expeditiously, we request that you complete the forms in this website. Please go to New Patient Forms and enter your password. (Password will be issued when appointment is made). Please complete the four forms listed. These completed forms can be sent to us electronically, thus allowing us to process your information promptly. Your completing these forms in advance will greatly speed up your evaluation process. If you cannot complete these forms on-line, then, upon your request, we will send them via US mail or email, whichever is most convenient for you. Having these forms completed and ready when you arrive for your first appointment is important, otherwise you will be asked to complete a set of forms upon your arrival. This may delay evaluations or may cause a need for rescheduling your appointment.

What to expect at your first visit:

It is very important that we learn as much about you as possible since pain treatment is done best in the context of understanding you well.

  • If your forms are completed prior to your first visit, you can expect a review of these forms upon your arrival. You may complete your forms online. Please go to New Patient Forms.
  • You will then meet with a case manager to take a thorough history, which will provide our professionals with important information. Some of this information will be obtained from the forms you filled out ahead of time.
  • You will also meet with: • One of our physicians • Our psychologist • Our physical therapist

We realize that you are in pain, and you will have the opportunity to eat lunch and take rest breaks during the evaluation process. If you have any special needs please make them known to the staff.

We occupy the entire first floor of Rubio Medical Plaza at 16550 Ventura Blvd. We are on the corner of Rubio and Ventura, on the south side of the street. You will be parking behind our building in the parking lot, and we validate parking for 4 hours. In many situations, it is unusual for you to be at the Center for greater than 4 hours, if this is your situation, please speak to the receptionist regarding how the parking is managed.
